Assessment Services

If you have been referred for a violence risk, sexual risk, or mental health assessment by the State of Maine Department of Health and Human Services (DHHS), the State of Maine Department of Corrections (DOC), or another entity, or if you would like to self-refer, please complete our assessment intake paperwork.

In addition, please complete a confidential release of information form, allowing Groundwork Healing, LLC to communicate with the referring entity and/or payor.

If you would like for Groundwork Healing, LLC to communicate with anyone else regarding your assessment, such as your attorney, guardian ad litem, external providers, etc., please complete separate release forms for each entity. Your clinician may request additional releases of information during the assessment process.

If you would like to refer a client for an assessment, please visit the ‘Assessment Referral Form’ tab to review the necessary steps (please do not complete intake paperwork on behalf of the client).
