Our intake paperwork is the first step in requesting individual or group services. It should be completed by the client seeking services or their legal guardian. A referral is not required.
Once your paperwork has been submitted, we will contact you within a few business days to confirm receipt and discuss next steps, including possible waitlist placement. Wait times vary depending on the service requested and clinician availability.
Assessment Intake Paperwork
The assessment intake paperwork is the first step in requesting an assessment. It should be completed by the individual seeking services or their legal guardian.

Assessment Referral Form
The Assessment Referral Form should only be completed by the referring agency, provider, or other authorized referral source. This form is used to refer an individual for an assessment and does not replace the client's intake paperwork.
Once the referral has been received, we will review the information and contact the client or legal guardian, as appropriate, to discuss next steps, including completion of intake paperwork and placement on the assessment waitlist, if applicable.
Release of Confidential Information
A release of confidential information form is a document that allows individuals to authorize the disclosure of their personal information, often medical or other protected information, to third parties. It ensures that the disclosure is done with the individual's informed consent, protecting their privacy and confidentiality.
